We bill insurance strictly as a courtesy to you. You are still responsible for payment if your insurance company is not covering the services rendered at our office.
We accept Tricare, in-network HMO’s, and PPO’s.
Please give us a 24 hour notice when cancelling appointments. If we are not notified 24 hours in advanced, a missed appointment charge of $10.00 will be applied to your account.
For newborns; after 1 month, newborns must have their own insurance and can no longer use parent’s insurance.
Co-Pays apply for all patients before every visit. (If needed)
Any account balances should be paid in full prior to services rendered.
